Requirements for Requesting Documents

Second Request of Transcript of Records

• Special Power of Attorney notarized at the place of your residence abroad with one (1) piece 2x2 inch. ID picture of representative (recent, not more than three months);
   
• Photocopy of your Passport with picture and signature;
   
• Representative’s valid Identification Card with picture and signature;
   
• Two (2) pieces of 2x2 inch. ID picture (recent, not more than three months), white background, formal pose in business attire (with collar);
   
• Duly Accomplished General Clearance Form (get blank form from W-116);
   
• Payment:
  • Non-engineering  graduates: Two Hundred Pesos (PhP 200.00)
  • Engineering graduates: Two Hundred Fifty Pesos (PhP 250.00)
  • Non-graduates: Fifty Pesos (PhP 50.00) per page

Certified True Copy of Transcript of Records or Diploma

• One (1) piece of 2x2 inch. ID picture (recent, not more than three months), white background, formal pose in business attire (with collar);
   
• Duly Accomplished General Clearance Form (get blank form from W-116);
   
• Provide Photocopy of Diploma or Transcript of Records;
   
• Payment: Fifty Pesos (PhP 50.00) per set of request.
