You may only file your requests via PUP Online Document Request System (ODRS). For Baccalaureate degrees (i.e., undergraduate), the Main Campus Registrar will start processing requests on October 14, 2024. Please upload the following requirements to immediately process your request:

Certificate of Candidacy
General Clearance
Receipt before graduation
- Proof of Payment for those not covered by RA 11032 – Free Education Law
- ODRS voucher for those covered by RA 11032 – Free Education Law
2x2 TOR Picture Wearing Toga

Requests with lacking requirements WILL NOT BE PROCESSED.

Original copies including PUP ID must be submitted upon claiming of your request.

This memorandum serves as a reminder regarding the preferred payment transfer methods to PUP Accounts.

eWallet Usage:
Please be advised that the Fund Management Office strongly discourages the use of eWallets (e.g., Gcash, Maya) for payments thru Fund Transfers to PUP Accounts. These platforms have system limitations that may lead to delays and require additional verification from Land Bank.
Online Banking Transfers:
When using online banking services other than Land Bank, please use PESONET as the third-party channel for payment transfers. This helps ensure timely processing and reduces the need for further verification, which could otherwise delay the issuance of official receipts.

Invitation to Submit Proposals/Quotation for Accreditation: Purified/Mineral Drinking Water Provider

The Polytechnic University of the Philippines (PUP), through the Resource Generation Office (RGO), invites interested parties to submit proposals/quotations for the supply of Purified/Mineral Drinking Water in PUP Main Offices, Sampaguita Canteen, and food concessionaires in PUP Sta. Mesa, Manila.

We require a certified provider to supply and deliver high-quality purified/mineral drinking water for our students, faculty, staff and events. The services should include:

Supply of Drinking Water:
- Purified and/or mineral drinking water
- Various packaging options (e.g., bottles, dispensers)
- Regular and emergency delivery services
Maintenance and Servicing:
- Regular cleaning and maintenance of water dispensers
- Immediate response to maintenance requests
Quality Assurance:
- ??Compliance with health and safety standards
- Regular quality testing and certification

Please include clear copy of the following documentary requirements:

Business Permit;
DTI Registration Certificate for sole proprietorship, SEC Registration for partnership and corporation, or Certificate of Registration (COR) for cooperatives Location map of the business establishment;
Tax Clearance/ BIR Registration Certificate;
Sanitary Permit;
Microbiological Analysis and Certification from Food and Drug Administration (FDA);
Bank Certification (to attest that the provider is in good financial standing) and latest financial statement issued by a CPA;
Location Map of the Business Establishment;
Two (2) valid government-issued ID of the registered business owner/s;
Notarized sworn statement that the service provider is not blacklisted or barred from any transaction with the government or any of its agencies, offices, corporations, or Local Government Units (LGUs);
Latest audited financial statement; and
Other pertinent detailed proposal/documents relative to operation including the following information:

a. Company Profile:
Background and experience in providing purified/mineral drinking water
Relevant certifications and accreditations
References from previous clients

b. Service Offering and Specifications:
Types and specifications of drinking water provided
Packaging options and sizes
Delivery and maintenance schedules

c. Pricing:
Detailed pricing for each type of water and packaging option
Any additional costs (e.g., delivery charges, dispenser rental)
Payment terms and conditions

d. Quality Assurance and Certifications:
Description of quality assurance processes
Copies of relevant certifications and recent quality test results

e. Customer Support:
Contact details for the service team
Emergency contact information

f. Sustainability Practices:
Eco-friendly practices in water bottling, packaging, and delivery

Terms of Reference for Purified/Mineral Drinking Water Provider

These Terms of Reference outlines the requirements and responsibilities for the provision of purified/mineral drinking water by the selected provider for PUP Main Offices, Sampaguita Canteen, and food concessionaires in PUP Sta. Mesa, Manila and Campuses.

Scope of Services
1. The Provider shall supply purified/mineral drinking water in various packaging options as required by the PUP.
2. Provider shall ensure timely and regular delivery of drinking water to the designated locations.
3. The Provider shall regularly maintain and clean all water dispensers and equipment supplied.
4. The Provider shall adhere to all relevant health and safety standards and conduct regular quality testing.
Service Requirements
1. The drinking water provided must meet all applicable health and safety standards and be free from contaminants.
2. Water should be available in a variety of packaging options suitable for different needs (e.g., individual bottles, large dispensers).
3. The Provider shall adhere to the agreed delivery schedule and accommodate emergency delivery requests when necessary.
4. The Provider shall regularly service and maintain any dispensers or related equipment provided to the Client.
Quality Assurance
1. The Provider must have all relevant certifications and comply with national and local health regulations.
2. Regular quality testing of the water must be conducted, with results available to the Client upon request.
3. The Provider must comply with all applicable environmental and health regulations.
Pricing and Payment
1. The pricing shall be as per the agreed quotation, with a detailed breakdown provided.
2. Payments will be made according to the agreed schedule and terms specified in the contract.
3. Additional costs must be clearly communicated and approved by the PUP prior to being incurred.
Cancellation and Termination
1. Either party may cancel the agreement with Thirty (30) days notice in writing.
2. The PUP reserves the right to terminate the agreement immediately in the event of a serious breach of terms by the Provider.
Liability and Indemnity
1. The Provider is liable for any harm caused by the drinking water supplied due to negligence or failure to comply with health standards.
2. The Provider agrees to indemnify and hold the PUP harmless from any claims, damages, or expenses arising from the provision of services.

All Proposal/Letter of Intent, Quotation, and Portfolio should be sealed separately and submitted to the Resource Generation Office (RGO) located at the 2nd Floor Dome, Main Building, PUP Mabini Campus, Anonas St., Sta. Mesa, Manila.
