You are hereby advised to upload online the following required documents:</p> <ol> <li>Certificate of Candidacy</li> <li>General Clearance (cleared in all offices)</li> <li>1 copy of 2x2 Colored picture in Toga with hood and cap (white background). Please see the attached required photo for your reference.</li> <li>Official Receipt of payment for those not covered by R.A. 10931 also known as Universal Access to Quality Tertiary Education Act of 2018 for the following: <ol start="1" style="list-style-type:lower-alpha"> <li>Transcript of Records Fee</li> <li>Certification of Graduation Fee</li> <li>Diploma Fee</li> <li>Graduation Fee</li> </ol> </li> </ol> <p>*Dummy diploma is no longer required.</p> <p>Please submit the ORIGINAL COPY OF THE DOCUMENTS upon release of your credentials.
