Advisory 26 s. 2020: For Applicants seeking Re-admission

Please submit a SCANNED COPY of the following documents:

1. Re-Admission Form (Reference Slip). Download a copy from the PUP Website – Downloads for Students tab.
2. Informative Copy of Grades. Request a copy through the Online Document Request System - odrs@pup.edu.ph.
3. Curriculum Sheet
4. 2” x 2” colored picture with white background and name tag on the photo (Last name, First name, Middle name).

Scanned copies should be emailed to re-admission.transferees@pup.edu.ph from August 17-20, 2020.

IMPORTANT NOTE: Submission DOES NOT MEAN immediate admission. We will still evaluate your credentials. Successful applicants will be informed of the schedule of enrollment.
