Senior High School Application for Incoming Grade 11 SY 2022-2023

Students who are currently in the DepEd K-12 program and are expected to move up to SHS this school year may be admitted in the Polytechnic University of the Philippines Senior High School (PUP-SHS).

Online Registration is from April 18, 2022 to April 24, 2022

Please make sure you have the following required documents ready for uploading during your online registration:

1. Scanned photo of the applicant (JPEG file, 2x2-inch recent picture with white background and applicant’s name tag at the chest)
2. Scanned Grade 9 Report Card/ F137 (JPEG file)
3. Scanned Grade 10 Report Card (JPEG file, 1^st and 2^nd grading periods)
4. ESC/QVR Certificate (a MUST for students coming from PRIVATE SCHOOLs who are currently enrolled as Grade 10 of School Year 2021-2022)

Important reminders, please read carefully.

• Report cards MUST CLEARLY show the applicant’s Complete Name, LRN, grades in English, Math, Science and General Average.
• Each JPEG file size must not be more than 300 kilobytes(KB)
• Use an active and correct email of the applicant.
• Once your online application is FINALIZED, NO more editing of application
• Multiple accounts, multiple applications are ground for INVALID REGISTRATION
• Wrong entry of information and grades are ground for INVALID REGISTRATION

Grade Requirements:

• English, Math and Science must be 85% and above
• General Average must be 85% and above
• No grade below 82% in other subjects

FAILURE TO UPLOAD PROPERLY THE REQUIRED DOCUMENTS MAY RESULT TO AN INVALID REGISTRATION

PUPSHS is a FIRST-COME-FIRST-SERVED basis, subject to availability of slots.
