Issuance and Validation of Library Cards

Who may avail of the Library Card?

• Bona fide PUP students;
• Faculty and Administrative personnel

Requirements in Securing a Library Card

• A library card application form (available at the NALLRC Main Library and its Satellite Libraries);
• Valid Registration card and/or Receipt;
• Current and valid 1 x 1 ID picture

Steps in Securing a Library Card

1. Secure and fill up library card application form available at the NALLRC Main Library (Readers Services) and its Satellite Libraries.
2. Present the filled-out library card application form to the library personnel assigned at the issuance of the Library Card.
3. After presenting the accomplished library card application, submit the same to the library personnel issuing the library card.
4. Fill out the Library Card form and proceed to the Office of the NALLRC Director located at the Center Wing of the NALLRC Bldg., for final signature.

Lost Library Card (Requirements)

• Valid Registration card and/or RECEIPT;
• Current 1 x 1 ID picture
• Affidavit of Loss (from the University Legal Office)
• Promissory note from Parent/Guardian
• Parent’s/Guardian’s valid / government ID

Lost Library Card shall be entertained after the Validation and Issuance schedules.

Library Card Color Codes

• Orange – Main Library (CAF, CAL, CB, CoED, CCIS, CHK, CPSPA, CS and CSSD)
• Violet – CEA, CAFA, COC and ITECH
• Green – Graduate School and CTHTM
• Blue – Law, OU, JHS and SHS
• Yellow – Admin and Faculty

Schedules

Validation of Library Cards (for those who already have a Library Card) is from June 20 to July 20, 2016, 9:00 to 11:30 AM, 1:30 to 4:30 PM.

Issuance of Library Card is from June 20 to July 20, 2016.

Issuance and Validation of Library Cards Extension
Senior High Schol: July 28 to August 5, 2016
Old students: August 8 to 12, 2016
For lost library card, duplicate copy issuance is from August 15 to 19.

Note: Library Card validation shall not be accommodated after the July 20, 2016 deadline. Lost Library Card shall be entertained after the Validation and Issuance schedules.
