Invitation to Submit Proposals/Quotations for Senior High School Uniform of Grade 11 Students in PUP Sta. Mesa, Manila for the School Year 2016-2017

The Polytechnic University of the Philippines (PUP), through the Resource Generation Office (RGO), invites interested suppliers/providers to submit proposals/quotations for the Senior High School uniform of Grade 10 students in PUP Sta. Mesa, Manila for the School Year 2016-2017.

Deadline for the submission of proposals is on 20 April 2016 (Wednesday). Sealed proposals/quotations together with the sample uniforms must be submitted at the RGO, 2^nd Floor Dome, Main Building, Polytechnic University of the Philippines, Sta. Mesa, Manila.

Proposal shall  include the following: Business Permit, DTI/SEC Registration,  BIR Certificate of Registration , Business/Establishment location map and photocopy of a valid government-issued ID of the registered owner/s. A. Scope of Work

1. Supplier to provide school uniform for approximately 6,000 students.
2. Supplier to follow the design of the school uniform as stated below:

   Polo for Boys
   Material: Tetoron
   Color: White
   Style: Polo jacket with PUP logo on the left side pocket

   Pants for Boys
   Material: cotton
   Color: navy blue 
   Style: slacks with side pocket
   
   Blouse for Girls
   Material: Tetoron
   Color: White
   Style: Blouse with 2 inches waistband, use navy blue shiny buttons
   
   Front - with navy blue piping on the sides of the collar with PUP logo   on the left side , necktie with print of PUPSHS with navy blue piping on the sides.
   
   Back -  with sailor-cut collar with navy blue piping on the sides
   
   Sleeves - with navy blue piping above the hemline

   Pants for Girls
   Material: cotton
   Color: navy blue
   Style: slacks with side pocket

B. School uniforms shall have the following sizes: extra small, small, medium, large, extra large and double extra large.

C. The supplier is responsible for the distribution of school uniforms to the students. A room for rent will be provided to the supplier.

D. Supplier shall provide sample sizes to facilitate measurement and distribution of school uniforms.

E. The purchase of school uniforms shall be on consignment basis.
