Junior High School Admission Evaluation for SY 2022-2023

Schedule of Online Registration: February 21 to April 15, 2022
After your online registration, the PUPLHS Evaluation Committee will evaluate your documents. Please be guided accordingly.

Who are Qualified for Admission?

Students who are currently in the DepEd K-12 program and are expected to move up to Junior High School (JHS) may be admitted to the Polytechnic University of the Philippines Laboratory High School (PUPLHS).

General Requirements

Grades from Grade 6 (First and Second Quarter)

Grade Requirements:

• English, Math, and Science must be 85% and above
• Average for each quarter must be 85% and above
• No grade below 82% in other subjects

Specific Requirements for Qualified Applicants

Please prepare the following documents before your scheduled date of enrollment:

1. Learner's Reference Number (LRN)
2. Accomplished Student Profile Sheet
3. Original Elementary SF-9 (Formerly F-138 or Report Card)
4. Original Elementary SF-10 (Formerly F137-A)
5. Certificate of Good Moral Character
6. Original PSA/NSO Birth Certificate
7. Health Information Sheet for Student
8. Two (2) pcs. 2x2 inch. photo on white background with name plate
9. One (1) pc. Long Brown Envelope
