Invitation to Submit Proposals/Quotations for the following products/services relative to the Mid-Year (December 2017) Commencement Exercises, Academic Year 2017-2018

The Polytechnic University of the Philippines (PUP), through the Resource Generation Office (RGO), invites interested providers/suppliers to submit proposals/quotations for the products/services relative to the mid-year (December 2017) commencement exercises for the academic year 2017-2018.

Around 1,000 students are expected to march in the mid-year commencement rites. These graduates are from PUP Main and other branches/campuses (Bachelor’s, Master’s and Doctorate levels).

Deadline for the submission of proposals is on or before 31 August 2017. Sealed proposals/quotations must be submitted at the RGO, 2^nd Floor Dome, Main Building, Polytechnic University of the Philippines, Sta. Mesa, Manila.

Proposals shall include clear photocopy of the following:

• Business Permit
• DTI/SEC Registration
• BIR Certificate of Registration/Tax Clearance
• Barangay Clearance
• Location Map of the Business Establishment
• Copy of two (2) valid government-issued ID of the registered owner/s
• Company/Business Profile (showing projects done)

A. Photo for Transcript of Records or TOR (Mandatory)

1. The photograph shall be colored, 2 pieces, 2 inches x 2 inches (51 mm x 51 mm)
2. The photo-print shall be clear and with a continuous-tone quality.
3. It shall have full face, front view (correct head size and head position, center head within frame, no hair in front of the hood)
4. The background shall be plain, without any distracting shadows on the face or on the background.
5. In .JPEG or .JPG file format
6. 600 x 600 pixels in dimension
7. Photograph shall be in high quality (no visible dot pattern, properly focused, non-discernible pixels)
8. Photograph shall have individual filename (e.g. Dela Cruz, Juan.jpg)
9. Soft/Digital copies of the photographs shall be saved in DVDs, 2 copies.
10. Printed copies of the photographs shall be delivered according to college/course.

B. Photo Services for Receiving of Diploma (Optional)

Specifications:

1. 2 pieces 5 inches x 7 inches colored picture with footer caption, printed in glossy paper
2. To be delivered to RGO according to college/course

C. Rental of Academic Gowns for the Graduating Students (Optional)
