Non-Traditional Study Program (NTSP) and Expanded Tertiary Education Equivalency and Accreditation Program (ETEEAP)

• Letter of application addressed to the University President (Dr. Emanuel C. De Guzman) through the Executive Vice President (Dr. Manuel M. Muhi) and the ETEEAP-NTS-TE Director (Dr. Raquel G. Ramos).
• Recommendation letters from three (3) notable persons in area of specialization.
• Educational Background
  • Original transcript of records for evaluation
  • Honorable dismissal from the school last attended
• Work experience (minimum of 5 years)
  • Service record or certification of employment
  • Appointment and/or designation with level of position and job description/duties and responsibilities per position held
  • Other proofs of work experience
• Two (2) pcs. latest Picture 2x2 (colored, photo taken on white background)
• Original Birth Certificate (NSO authenticated)
• Description of subjects taken from previous school/university (for evaluation purposes).

Please submit the following documents for accreditation and equivalency (NTSP and ETEEAP):

1. Innovation, Invention, Research and Publication
   1. Application for or actual patent or copyright
   2. Manuscript or publication of researches
   3. Sample of book, workbook, module, manual, monograph, article or creative work
   4. Instructional/audio visual material developed
   5. Copy of unpublished research
2. Expert Service, Training and Active Participation in Professional Development Activity
   1.  Certificate or contract of consultancy and/or advisorship
   2. Training certificate
   3. Certification or plaque as coordinator, lecturer, resource person, guest speaker, etc.
   4. Skills certification from TESDA
   5. Other proofs of expertise
3.  Conference, Seminar, Workshop, Colloquium or Forum Attended
   1. Certificate of attendance or appearance
   2. Receipt of payments for attendance, if applicable
   3. Other proofs of attendance
4. Officership/Membership in Relevant Organization
   1. Certificate of membership, receipt or payment of membership fee or ID
   2. Other proofs of membership
5. Community Outreach
   1. Written assignment in organized community project duly assigned by authorized official
