Invitation to Submit Proposals/Quotation for Accreditation: Purified/Mineral Drinking Water Provider

The Polytechnic University of the Philippines (PUP), through the Resource Generation Office (RGO), invites interested parties to submit proposals/quotations for the supply of Purified/Mineral Drinking Water in PUP Main Offices, Sampaguita Canteen, and food concessionaires in PUP Sta. Mesa, Manila.

Proposal/Letter of Intent and Quotation addressed to Dr. Annabelle A. Gordonas, Director, Resource Generation Office (placed in a separate sealed envelope) must be submitted to the Resource Generation Office (RGO), 2nd Floor Dome, Main Building, PUP Mabini Campus, Anonas St., Sta. Mesa, Manila.

Proposal/Letter of Intent shall include clear photocopy of the following documentary requirements:

Business Profile
City and Barangay/Business Permit
Sanitary Permit
Microbiological Analysis and Certification from Food and Drug Administration (FDA)
BIR registration and Tax clearance
Bank Certification (to attest that the provider is in good financial standing) and latest financial statement issued by a CPA
Registration Certificate from SEC for Corporation, DTI for Sole Proprietorship, or CDA for Cooperatives
Location Map of the Business Establishment
Two (2) valid government-issued ID of the registered owner/s and other pertinent documents required
Sworn statement that the supplier is not blacklisted or barred from any transaction with the government or any of its agencies, offices, corporations, or Local Government Units (LGUs)

Scope of work/Terms of reference:

Supplier is responsible for the distribution of Purified/Mineral Drinking Water in PUP offices, University Canteen, and food concessionaires in PUP Sta. Mesa, Manila.
Supplier is responsible for the provision of hot & cold water dispenser in PUP offices in PUP Sta. Mesa, Manila.
