Students applying at the PUPSHS should present proof of their status as Qualified Voucher Recipient (QVR) for admission to the PUPSHS:

1. For students coming from public schools – Junior High School completion certificate or report card
2. For ESC grantees from private schools – ESC certification letter from school principal
3. For qualified SHS voucher applicants from private schools – Qualified Voucher Recipient (QVR) certificate; students may verify the status of their Voucher Application and the process for acquiring a QVR Certificate at the Online Voucher Application website (http://ovap.deped.gov.ph)
