Procedures on How to Avail of the UCCA Services

PUP UCCA Entrance Artists

General Requirements:

•  Certification from the School Principal (with dry seal) of the exemplary performance of the applicant
•  Certification of Good Moral Character
•  PSA Birth Certificate
•  High School Card (Form 138) with a minimum General Weighted Average of 85%
•  Commencement Exercises Program List of Graduates
•  PUPCET Stub (non-PUPCET takers will be advised to take the exam)
•  Portfolio of Achievements in the field of culture and the arts
•  2 pieces of 2x2 photo with name plate and white background.
•  Long Brown Envelope

Upon enrolment, all Documents must be presented to the UCCA.

•  Affidavit – Waiver (duly signed by the applicant and notarized by a lawyer)
•  PUPSAIT Result (Non-PUPCET takers)
•  Copy of Registration Card

1. Present the original and photocopy of the documents to the Scholarship and Financial Assistance Services (SFAS) for validation and evaluation
2. Proceed to UCCA for screening and submission of a portfolio.
3. Secure Scholarship Agreement Form from SFAS Office (For Non-PUPCET Takers) Take the PUP Scholastic Aptitude and Interest Test (PUPSAIT) on the scheduled date and time.
   3.1. (For Non- PUPCET Takers) Take the PUP Scholastic Aptitude and Interest Test (PUPSAIT) on the scheduled date and time.
4. Proceed to the ARO for credential processing, interview, submission of the original and three (3) photocopies of the ARO requirements to secure admission slip for enrollment.
5. Proceed to the college to check the availability of slots.
6. Proceed to ICTO for tagging in the Student Information Service (SIS).
7. Secure medical clearance.
8. Secure printed copy of registration certificate.

Cultural Performance

•  Doxology songs or performance
•  Singing of the Philippine National Anthem
•  Intermission Numbers/Performances

For Internal Clientele (PUP Community)

Requirements:

•  Invitation Letter
•  Program
•  Food
•  Certificate of Appreciation
•  Transportation

1. Submit letter of invitation addressed to the UCCA Director stating the nature of event, venue, date, time and specific request of cultural performance one week before the actual day of event.
2. Have the letter duly signed for approval by the respective Cultural Section Chief and endorse to the UCCA Director subject to the availability of the members of the invited cultural group.
3. Secure an approved Agreement Form duly noted by the UCCA Director.
4. The UCCA Office shall secure excuse letters and waiver for the student artists who are members of the invited organizations.
5. Provide program for UCCA group as reference.

For External Clientele (Outside PUP)

Requirements:

•  Invitation Letter
•  Program
•  Food
•  Certificate of Appreciation
•  Transportation
•  Dressing Room

1. Submit letter of invitation addressed to the PUP President stating nature of event, venue, date, time and specific request of cultural performance two weeks before the actual day of event. Please indicate in the letter if the invited cultural group would be given a token or honorarium.
2. Should the Office of the President approved of the request, the letter would be forwarded to the UCCA Office by the Office of the Vice President for Student Affairs and Services.
3. Status of the letter may be followed-up at the UCCA Office. Have the letter duly signed for approval by the respective Cultural Section Chief and endorse to the UCCA Director subject to the availability of the members of the invited cultural group.
4. The UCCA Office shall coordinate with the inviting party regarding the details of the event.
5. The UCCA Office shall secure waivers for the student artists who are members of the invited organizations.
6. Provide program for UCCA Office as reference.

Use of the Tanghalang PUP Theatre Facility

A.  For the PUP Community

1. Submit letter of request addressed to the UCCA Director through the Theatre Manager stating nature of event, venue, date and time two weeks (10 Working Days) before the actual day of event and indicate whether there are sponsoring agencies for the event. No e-mail reservations will be entertained. (Please see prescribed letter format)
2. Have the Theatre Manager note the availability of the requested date/s and endorse it to the UCCA Director for approval.
3. Secure a Theatre Lessee Agreement Form from the UCCA Office.
4. Once the date is approved, provide the UCCA Office a copy of the following:
   4.1  Photocopy of ID and contact details of the requesting party.
   4.2  Program of the Event.
   4.3  Stage Design Layout.
   4.4  Lobby Design Layout.
   4.5  Sponsorship Details (if any) - size and nature of booth, electrical needs, product/s to sell, flyers to give-away, etc.
   4.6  Supplier Details (if any) - inventory of lights and sounds equipment and contact details of the supplier. (Note: Services from outside supplier will not be granted the use of lights and sounds equipment in the theatre.)
5. Bring the approved request letter and agreement form to the RGO and secure a Routing Slip.
   5.1 Secure from the Facility and Management Office (FaMO) the computation of the energy fee. Clarify whether the requesting party will be using the available equipment of the theatre or will be hiring services from an outside supplier. The complete inventory list of lights and sounds equipment should be given to FaMO for the cost of the electrical consumption to avoid unnecessary electrical fluctuations, failures, or fire.
6. Pay the prescribed amount (Energy Fee + Venue Fee) at the PUP Cashier and get the PUP Official Receipt. (Note: Only PUP Office of the President can issue or notify the free or waived payment use of the theatre)
7. Submit a photocopy of the PUP Official Receipt or duly accomplished Routing Slip with waived payment to the UCCA Office two (2) working days before the reserved date.
8. Requesting Party should provide the PUP Security Office a copy of accomplished Routing Slip with attached Letter Request addressed to UCCA (with note of the availability of requested date/s), complete details of the supplier and equipment, for entry to PUP premises of resource speakers, guests, audience and ingress and egress of equipment by the production team.

B.  Outside Lessees

1. Submit letter of request addressed to the UCCA Director through the Theatre Manager stating nature of event, venue, date and time one month (20 Working Days) before the actual day of event and indicate whether there are sponsoring agencies for the event. E-mail reservation will be entertained but with proper phone call and actual coordination with the UCCA Office before the requested date/s is/are reserved. (Please see prescribed letter format)
2. Have the Theatre Manager note the availability of the requested date/s and endorse it to the UCCA Director for approval.
3. Secure a Theatre Lessee Agreement Form from the UCCA Office.
4. Once the date is approved, provide the UCCA Office a copy of the following:
   4.1  Photocopy of ID and contact details of the requesting party.
   4.2  Program of the Event.
   4.3  Stage Design Layout.
   4.4  Lobby Design Layout.
   4.5  Sponsorship Details (if any) - size and nature of booth, electrical needs, product/s to sell, flyers to give-away, etc.

Note:

•  In case the Lessee’s (whether a PUP unit or Outside Lessee) intention is an Income Generating Project (IGP), or a Fund-Raising Activity (FRA), Letter Request Reservation of the Tanghalang PUP should be addressed to the UCCA Director while arrangements should also be made through the Resource Generation Office (RGO) on the details concerning the project. Lessees should comply strictly with the Internal Audit Office policies.
•  On weekdays, Tanghalang PUP opens at 8:00AM, thus the lessees shall begin their ingress at this time. Events should end at 6:00PM and egress shall be completed by 8:00PM.
•  There are no reservations on a Saturday unless authorized by the PUP Office of the President.
•  Reservations on Saturdays and Sundays should be made through the PUP Office of the President.
•  Reservations shall only be final once the lessee presents to the UCCA Office the PUP Official Receipt of payment three (3) working days prior to the event for Internal Clients (PUP Community) and five (5) working days prior to the event External Clients (Non-PUP Organizations).
