Application Requirements

Submit via email the following scanned copy of the requirements in a .zip or .rar file to ouspbditadmission@pup.edu.ph: Kindly use this template as your email subject: PSYE_Desired Program_Surname (example: PSYE_PBDEET_DelaCruz).

1. Letter of Intent addressed to the PUP OUS Executive Director, Dr. Carmencita L. Castolo
2. Filled-up Recommendation letter form (Download the form)
3. Transcript of Records from the last school attended
4. PSA Birth Certificate
5. PSA Marriage certificate (for female applicants who opt to use her husband surname)
6. Certificate of Employment
7. NBI/Police Clearance (for unemployed applicants)
8. PUP iApply payment voucher
9. Proof of payment (duly validated LBP Cash Deposit Slip or PUP Cashier Official Receipt)
10. Two copies ID pictures, 2 x 2 inch (recent, identical, in formal attire, white background)

Php 200.00 per unit for the PBDIT Program
Miscellaneous fees: Php 1,700.00
