Who is Qualified for Admission?

Students currently in the DepEd K-12 program and are expected to move up to Junior High School (JHS) this School Year 2026-2027 may be admitted to the Polytechnic University of the Philippines Laboratory High School (PUPLHS).

General Requirements

Grades from Grade 6 (First and Second Quarter)

• ENGLISH, MATH, and SCIENCE must be 85% and above
• General Average must be 82% and above
• No grade below 80% in other subjects

Specific Requirements for Qualified Applicants

Please prepare the following documents before your scheduled date of enrollment:

1. Learner’s Reference Number (LRN)
2. Accomplished Student Profile Sheet
3. Original Elementary SF-9 (Formerly F-138 or Report Card)
4. Original Elementary SF-10 (Formerly F137-A)
5. Certificate of Good Moral Character
6. Original PSA/NSO Birth Certificate
7. Health Information Sheet for Student
8. Two (2) pcs. 2x2 inch. photo on a white background with nameplate
9. One (1) pc. Long Brown Envelope

Admission Policy

The following are the policies regarding admission into the PUPLHS:

1. No student shall be denied admission into the PUPLHS because of age, sex, nationality, religious belief, ethnicity, or political affiliation.  NO TRANSFEREES, however, shall be accepted on all levels.
2. Applicants seeking admission must comply with the GENERAL, SPECIFIC, and GRADE requirements.
3. Admission to PUPLHS is granted to qualified applicants with a definite understanding that the student and his/her parents/guardian agree to comply with the rules and regulations of the PUPLHS.

Online Registration

Please make sure you have the following documents ready for uploading during your online registration:

• Scanned photo of the applicant (JPEG file, read photo guidelines)
• Scanned Grade 6 Report Card (JPEG file, 1st and 2nd grading periods)
• Report Cards MUST CLEARLY show the applicant's complete name, LRN, grades in English, Math, Science, and General Average
• Each JPEG file size must not be more than 300 kilobytes (KB)
• Active and correct email address of the applicant
